***PLEASE NOTE THERE IS A NEW SPORT FORM THAT IS REQUIRED FOR SPORTS. THE OLD FORM (from last year) IS NO LONGER ACCEPTABLE***

All forms may be found on the district website, under parents, then medical forms.

A current physical exam must be on file (must be dated within one year of the start of the sport). All physicals must be on a NYS School Health Examination dated after November 1, 2022.

 Complete and sign the 2-sidedSports Health History & Consent Form”.

READ FORM CAREFULLY and follow instructions below to ensure your child can be cleared.
This form must be dated within 30 days of the start of the sport.

  1. Complete entire form in INK
  2. Sign and date on page 2
  3. Explain ALL yes answers in section provided.
  4. Only 1 sport may be chosen per season.
  5. Student name must be on both
  6. Date form completed is the date this form is completed and must be date after October 6, 2023.

Sports Health History & Consent Form

For use of an EpiPen or Inhaler, a Parent and Pediatrician must fill out an “Authorization for Administration of Medication in School” form (found on our website). Parts A, B and C must be completely filled out.  This form is only valid for one year. Any student with an allergy or asthma must have a self-carry order or note from a doctor advising medication is not required. You will not be cleared without this form. Please hand in the form no later than 9:00 am on November 3 or your child will not be cleared in time. Late try-outs are at the discretion of the Athletics department and/or coach and may not be permitted.
