All forms may be found on the district website, under parents, then medical forms.
A current physical exam must be on file (must be dated within one year of the start of the sport). All physicals must be on a NYS School Health Examination dated after January 1, 2023.

Complete and sign the 2-sided “Sports Health History & Consent Form”. READ FORM CAREFULLY and follow instructions below to ensure your child can be cleared. This form must be dated within 30 days of the start of the sport.

For use of an EpiPen or Inhaler, a Parent and Pediatrician must fill out an “Authorization for Administration of Medication in School” form (found on our website). Parts A, B and C must be completely filled out. This form is only valid for one year. Any student with an allergy or asthma must have a self-carry order or note from a doctor advising medication is not required. You will not be cleared without this form. Please hand in the form no later than 9:00 am on January 12 or your child will not be cleared in time. Late try-outs are at the discretion of the Athletics department and/or coach and may not be permitted.

Todos los formularios se pueden encontrar en el sitio web del distrito, debajo de padres y luego en formularios médicos.
Se debe tener archivado un examen físico actualizado (debe tener fecha dentro del año posterior al inicio del deporte). Todos los exámenes físicos deben realizarse en un examen de salud escolar del estado de Nueva York con fecha posterior al 1 de enero de 2023.
Complete y firme el “Formulario de consentimiento e historial de salud deportiva” de dos caras.
