Medical Office Sports Clearance Process:

All students who intend to participate in Interscholastic Sports must:

Have a current physical exam on file dated within one year of the start of the sport. You can upload the completed Physical Examination Form to the Final Forms Sport Registration Portal. Please make sure your child’s pediatrician checks the box “Family Cardiac History Reviewed” on the physical exam, page 2. This is in reference to the Dominic Murray Sudden Cardiac Arrest Prevent Act and is required for your child to be cleared.

For use of an EpiPen or an Inhaler, a Parent and Pediatrician must fill out an “Authorization for Administration of Medication in School” form (found on our website). Parts A, B and C must be filled out. This form is only valid for one year. You will not be cleared without this form.
