Immunizations:  
New York State Department of Health requires the following immunizations for students entering the 6th grade.  Please note that new immunization requirements are in bold.

Tdap after age 10 (Tetanus, Diphtheria, and Pertussis) Documentation of the immunization is required within 14 days of their 11th  birthday.

Polio (unless the 3rd shot was given at/after age 4)

Varivax vaccine (Chicken Pox) & MMR (Measles, Mumps, Rubella)

Hepatitis B vaccine

DTaP  (unless the 4th was given at age 4 or older)

Blood work proving immunity is also acceptable documentation for Varivax, MMR, & Hepatitis B only. 

Medication:       
Medications to be taken in school must be in the original container and brought to the Medical Office with written parental consent and a signed doctor’s order dated after 6/30/2024. Students are only allowed to self-administer Epi-Pen and Inhalers provided the doctor’s order allows self-carry. All over-the- counter and topical medications require a doctor’s order, (which is valid for one year) and must be kept in the medical office in the original container.  Please consider Tylenol or Advil for children prone to headaches, cramps, or after the placement or adjustment of braces. All medication must be picked up at the end of the school year or they will be discarded.
 

Allergy:             
Please notify the medical office at the beginning of the school year about any food, insect or drug allergy. Please inform the Nurses of what occurs during a reaction and what the first course of action is: Benadryl, Epi-Pen, 911, etc. Medication orders and/or Food Allergy Action Plan is required.

Medical Conditions: Please notify the medical office at the beginning of the school year about any medical conditions (such as asthma, diabetes, seizures, cardiac issues, etc.). 

Injuries:  Should an injury occur, please notify the Medical Office right away. A student must have an MD note for any apparatus worn/used (Cast, ace bandage, brace, splint, boot, sling, crutches, or a wheelchair)A Physician’s medical note is REQUIRED if a student cannot participate in PE, requires the use of the elevator, has crutches, or requires a scribe. Students using crutches MUST wear full supportive footwear.  Slides/sandals/clogs etc are not permitted while using crutches. A doctor’s note is also required to return to PE if no end date was on the original note.  Documentation is required as per NY State Law.

Field Trips:  To participate, any student with asthma or allergies must have either a valid inhaler or epi-pen order filed with the nurse and medication in the health office if the order does not allow for a self-carry permit OR a note from the doctor stating the student no longer requires rescue medication for asthma or allergies.  

Sick Calls:  Parents must call the House Office if a student is absent because of illness.

Contact Info   Please contact the House Office immediately to change any Contact Information: new home, work, or cell phone so parent/guardian can be contacted right away. Please notify the House Office and Nurse if parents are traveling and the student is being cared for someone else.  

Communicable Diseases:      If your child has been diagnosed with the Flu, Covid 19 or RSV, it must be reported to the nurse.  Please review the guidelines for keeping children home from school with communicable diseases, fever, vomiting.  Guidelines can be found on the Medical Office webpage.  Guidelines for Keeping Children Home 

Pacers/Mile Run:   Please contact the PE instructor if your child has asthma, requires an inhaler prior to activity, or can only walk the mile. If PE is before lunch, please be sure to eat a healthy breakfast and are well hydrated.
